Add Student Personal Information

EXPLANATION OF PAGE

The Student Personal Information Page allows you to enter all necessary personal information about the selected student.  The fields on this page have been selected by the School System as a means of collecting the information needed to complete an IEP (or other documents) and to provide them with necessary means of tracking and querying special education data.

DETAILED PAGE INSTRUCTIONS

The Student Personal Information Page contains most of the information that is required by state and federal reports.  The student's contact information (address, phone, etc.) may be found on the Parents page.  Each student is required to have a unique Student Identification Number (Student ID).  EasyIEPTM will not allow a new student to be added with the same Student ID as an existing student.  Some School Systems use the student Social Security Number (SSN) as the StudentID.  If this is the case, then the SSN should be entered as the Student ID.  School Systems that use their own Student ID numbering system may also track the students SSN by entering it in the SSN edit field.

HINTS

  • If a student's school is changed, that student will not appear on any teacher's list until the student is assigned to a teacher in the new school.

    • WARNING

    No information (data) will be entered into the database unless the Update Database Button has been clicked after you have entered your data on the page.
