EXPLANATION OF PAGE

The Eligibility Page allows you to enter all eligibility information and allows access to the Select Evaluation Components area. From here, you may select the different evaluation categories for the student (i.e. Medical, Physical, etc.). Once these categories are added, the date for the completion of the evaluation can be entered.

The Eligibility Page allows for the tracking and documentation of the Eligibility process.  When the student was referred to Special Education department to be considered as a possible Special Education Student, the date of this referral is entered via the Eligibility Page.  This starts the Special Education time line that tracks important dates.

DETAILED PAGE INSTRUCTIONS

On this page, you may enter information concerning the Child Study Source, Referral Source, Dates for Child Study, Referral, Eligibility of Student, and all disabilities. Evaluation Components used in the eligibility process are collected from the list used by the School System.  Other components can be added or deleted as needed by School System administrators.

  • The components of the student's evaluation for participation in Special Education are selected by first clicking on the Select Evaluation Components Button and checking all of the appropriate Evaluation Components on the Select Eligibility Evaluation Components Page.
  • Once the Update the Database Button is pushed, you are taken back to the Eligibility Page where you will see that the new Evaluation Components have been added to the list.
  • Once the Eligibility decision is made, it is documented by setting the date of the decision and selecting Yes or No from the Eligible? drop down list.  Once the student is determined to be Eligible, he becomes an Active student.
  • The Child Study Source is the source that recommended the selected student for Special Education.
  • The Child Study Date is the date the selected student has been referred to the Child Study Team (or other appropriate team).
  • The Referral Date is the date the process for determining eligibility begins.
  • The Eligibility Date is the date the decision for eligibility was made.
  • The Referral Source is the source that referred the student.
  • The Eligibility Date is used by EasyIEPTM to track the eligibility of the students and to allow the notification of deadlines in the IEP compliance process.

    No information (data) will be entered into the database unless the Update the Database Button has been clicked after you have entered your data on the page.
