Add Custom Assessment

EXPLANATION OF PAGE

The Add Custom Assessment Page allows you to type-in custom assessments that would not be found on the set of listed assessments provided in the program.  This will give you the flexibility to add unique assessments for specific student needs.


DETAILED PAGE INSTRUCTIONS

  • After clicking on the Add Custom Assessment Button on the Assesment Page, you will be taken to the Add Custom Assessment Page.
  • Click on the Assessment Type text field and enter the type assessment.
  • Click on the calendar icon and enter the date administered or type the date in the text field in correct format (mm/dd/yy).
  • Enter the Subject Areas Tested by clicking in one of the text fields and typing the information.
  • Click on the checkboxes to select the Scores Reported.
  • Once these have been selected and theAdd Assessment(s) to Database Button is clicked, you will be taken to the Test Results Page where you may enter a narrative (comment) regarding the test, Add Subject Areas, and Select Score Types.

  • HINT

    If the assessment can be used by others in the School System, E-Mail your Special Education Administrator to have the assessment type added to the master dropdown list so all teachers can benefit from it.


    WARNING

    No information (data) will be entered into the database unless the Add Assessment to Database Button has been clicked after you have entered your data on the page.

Should there be a relationship between the goals and objectives in the IEP and those that are in instructional plans of special education personnel? When must IEP objectives be written before placement or after placement? Can short term instructional objectives be changed without initiating another IEP meeting? Must the IEP include all special education and related services needed by the child or only those available from the public agency? Is the IEP a commitment to provide services i.e., must a public agency provide all of the services listed in the IEP? Must the public agency itself directly provide the services set out in the IEP? Does the IEP include only special education and related services or does it describe the total education of the child? If modifications are necessary for a child with a disability to participate in a regular education program, must they be included in the IEP? When must physical education (PE) be described or referred to in the IEP? If a student with a disability is to receive vocational education, must it be described or referred to in the students' IEP? Must the IEP specify the amount of services or may it simply list the services to be provided? Must the IEP of a child with a disability indicate the extent that the child will be educated in the regular educational program?
